Predictors and outcomes of acute, sub-acute and early stroke following transcatheter aortic valve implantation

چکیده
Abstract Background Stroke is one of the most devastating complications after transcatheter aortic valve implantation (TAVI). The recent third Valve Academic Research Consortium (VARC-3) proposes new stroke terminology according to time between TAVI and onset. Purpose We aimed identify predictors assessed mortality in patients undergoing transfemoral complicated by acute, sub-acute early stroke. Methods Patients were included a global patient level database. Acute was defined as occurring ≤24 hours index procedure. Sub-acute >1 day ≤30 days >30 ≤365 following TAVI, VARC-3. identified for these using multivariate logistic regression analysis outcomes patients. Results A total 11230 underwent TAVI. Mean age 81.5±7.0 years, 58% female median STS-PROM score 6.5% (4.0%-13.2%). 405 (3.6%) experienced during first year Of 93 (23%) had acute stroke, 195 (46%) 117 (27%) One highest (56.9%), followed (41.7%), (29.0%), but one-year all higher than non-stroke (40.5% vs 15.8%, p<0.001). Glomerular filtration rate an independent predictor (odds ratio [OR] 0.9, 95% confidence interval [CI] 0.9–1.0, p=0.03). Previous cerebrovascular events independently predicted (OR 2.1, CI 1.4–3.1, p=0.001). Independent 1.0 per year, 1.0–1.0, p=0.04) peripheral vascular disease 2.0, 1.4–3.0, Conclusions showed Earlier timing post-TAVI associated with increased mortality. renal impairment; previous disease, suggesting different pathways leading Funding Acknowledgement Type funding sources: Foundation. Stroke following transcatheter aortic valve implantation. Is neuroprotection justified?
Degenerative aortic stenosis (AS) is the most frequent type of valvular heart disease. In patients with symptomatic AS surgical aortic valve replacement (SAVR) is a recommended treatment strategy. Due to a high risk of perioperative mortality, up to 30% of patients with AS are considered not suitable for SAVR. In the last 10 years dynamic development of transcatheter aortic valve implantation (...
متن کاملCognitive Outcomes following Transcatheter Aortic Valve Implantation: A Systematic Review
Severe aortic stenosis is the most common valvular heart disease in the elderly in the Western world and contributes to a large proportion of all deaths over the age of 70. Severe aortic stenosis is conventionally treated with surgical aortic valve replacement; however, the less invasive transcatheter aortic valve implantation (TAVI) is suggested for those at high surgical risk. While TAVI has ...
متن کاملAtrial Fibrillation Following Transcatheter Aortic Valve Implantation
The issue of atrial fibrillation (AF) following TAVI may seem of limited interest at first sight, and this is probably the reason why this issue has not been addressed in depth so far. First, AF is usually a more important concern in mitral than in aortic valve diseases. Second, the occurrence of AF following surgery under cardiopulmonary bypass is generally interpreted as a consequence of peri...
متن کاملCoronary Obstruction Following Transcatheter Aortic Valve Implantation
BACKGROUND Transcatheter aortic valve implantation (TAVI) was established as an important alternative for high-risk patients with severe aortic stenosis. However, there are few data in the literature regarding coronary obstruction, that although rare, is a potentially fatal complication. OBJECTIVE Evaluate this complication in Brazil. METHODS We evaluated all patients presenting coronary ob...
متن کاملTranscatheter aortic valve implantation complicated by acute pericardial tamponade
Transcatheter aortic valve replacement (TAVR) is now an accepted standard of care for patients with symptomatic severe aortic stenosis who are not candidates for surgery or have high surgical risk. Despite its more widespread adoption as a treatment option and increasing experience of centers, TAVR is still associated with several complications. We therefore report a case of TAVR complicated by...
